Why is Home Health needed?

Home Health is the proactive approach to healthcare. Our goal is to re-establish you and your family members independence. We want to get you to the point where eventually you don’t need us.

What is needed to get started?

A doctor’s order, demographics/insurance information, and the most recent progress notes from your provider (history and physical).

What to Expect?

Our team will reach out to pre-screen and schedule your initial appointment. Upon the first evaluation from our licensed home health staff, a care plan that is specifically tailored to you will be developed to with your doctor and established. The frequency and duration of Home Health visits clinically recommended will be communicated so that everyone is on the same page.

Who pays for Home Health?

Medicare, and most private insurance covers 100% of home health services. We will be more than happy to look up your coverage.

Who is eligible for Home Health?

A patient must have a skilled need, and be considered “Homebound” – a taxing effort to get from one place to another.

How Long can Someone Stay in Home Health Care?

An Initial Doctor’s Order is good (if needed) for up to 2 months of home health services. If the goals set for Patient at the initial assessment have not been met, and clinically the patient can benefit from additional services – an additional doctor’s order can be written, a recertification of services etc.
